JOB TITLE: PERSONAL CARER
GENERAL DESCRIPTION: Personal Carers assist our clients
with their personal needs in order for them to remain living in their
own homes.
REPORT TO: Team Leader
RESPONSIBLE FOR:
• Supporting the maintenance or restoration of the client’s
independence
• Assisting the client with their personal care needs
• Ensuring the client maintains adequate hydration and nutrition
• Supporting the interests, needs and rights of the client
• Performing household tasks as required
• Monitoring the change in the client’s condition and needs
• Maintaining effective communication
ACCOUNTABLE
FOR:
• Observe and record the limitations of the client
• Encourage the client to perform tasks as per the Care Plan
• Ensures the client is clean and comfortable
• Provides personal assistance according to the needs and wishes
of the client
• Assists with walking and transfers as required
• Assists client to take medication as per Webster pack and Nursing
Book
• Planning, preparing and serving nutritious meals
• Assists with feeding and drinking when required
• Assist
client with emotional, psychological, cultural and spiritual needs
• Assists with celebrations and special events
• Transport client to outings as required
• Demonstrate respect for the rights, confidentiality and dignity
of clients and their families
• Perform all light housekeeping and laundry duties as needed
within the client's environment.
• Immediately report any changes in client’s condition or
incidents to Caregivers.
• Observe and record in communications book any perceived changes
in the client’s needs. If urgent contact Caregivers
• Carry out instructions from Team Leader and Care Plan
• Complete all documentation as required, ensuring what is written
is brief, factual and not diagnostic
• Complies with Caregivers policies and procedures.
QUALIFICATIONS & EXPERIENCE:
• Certificate III in Community Services
• Senior First Aid Certificate
• Current CPR Certificate
• Police Check clearance
• Previous experience in personal care duties
• Ability to relate to older people with empathy and understanding
• Ability and willingness to follow written and verbal instructions
• Commitment to work as part of a team
• Understanding of privacy and confidentiality requirements
• Telephone accessibility
• Current drivers license and availability of an appropriate vehicle
with current comprehensive insurance
• Ability to use initiative and work independently whilst being
aware of sharing responsibility
